If you are injured at work it is important to let us know immediately.
If you are in need of an ambulance or will bring yourself to the doctor or emergency room, please tell someone before you leave.
When you arrive at the facility, please let them know that this is a Worker's Comp claim and that our insurance is with Wright Risk Insurance Group.
Once you have been set up, Wright Risk will issue a claim number to you and the providers can use that number.
You must complete C-3 and C-3.3 forms and return them to Human Resources as soon as possible.  This is how a claim number is formed.
